Have you filed for graduation? Please complete a DIPLOMA INFORMATION FORM according to the schedule below. If a Diploma Information Form is not submitted, your academic record will NOT be reviewed by your Major Department for graduation certification, nor by the Office of the Dean of Student Affairs for certification of the General Education and total degree requirements.
